Our wide range of disability services caters to diverse needs, including complex high care, high-intensity supports, private in-home care and nursing services, social support, assistance with daily living, and community access. We provide you with hospital-level care in the comfort of your own home, helping you maintain your independence and focus on what matters most to you and those who matter most to you.

Specialist Disability Support Services

Start your journey with us

First, you need to make sure we’re the right organisation for you and your child.

  • Participants across different ages from new borns and infants to you adults and 65-years
  • Participants with a current NDIS plan that’s NDIA or Plan Managed
  • Clients who have a Public Trustee, Private Trustee or have a claim with a state-based insurance scheme (ICWA and TAC)
  • Participants who require 30 or more hours of supports a week
  • Participants who require supports with assisted with self-care activities, supported independent living (SIL), access to community, social and recreational activities, for standard and high intensity care.
  • Participants who require nursing supports delivered by an Enrolled Nurse, Registered Nurse, Clinical Nurse, Clinical Nurse Consultant, or Nurse Practitioner.
  • Participants who have been diagnosed with an acquired brain injury, spinal cord injury, children with complex care needs, and neurological conditions with limited physical capabilities.

We train our team in specific skills to ensure safe and high-quality services. This sometimes means we’re not the right fit for everyone, and that’s okay. Unfortunately, we may not be best placed to support individuals with aged care packages or individuals with a primary diagnosis of intellectual disabilities or mental health conditions.

As an official provider of the Ventilator Dependent Quadriplegic Community Care Program (VDQ-CCP) in WA, we are dedicated to supporting you at every step.

We specialise in providing nursing and home care services to adults, teens, and children with Acquired Brain Injuries (ABI). We aim to provide personalised care that helps maximise independence and allows our clients to live as they choose.

As an ABI care provider, we ensure that our care team is well-equipped with the necessary knowledge and resources to support individuals struggling with physical, cognitive, emotional, or behavioural functions. We achieve this through consistent learning and mentoring opportunities for our team.

Support for infants, children, and families with complex care needs.

Children with complex medical needs require 24/7 care and attention. Our dedicated team of Paediatric Nurses and qualified care team provides high-level support to children in their homes. We aim to keep the child out of the hospital and in the safe familiarity of their own home.

Staff are trained to meet the needs of each child through specific clinical training carried out by our Paediatric clinical nursing team. This training includes Tracheostomy and Ventilator Management, Seizure Management, Enteral Feeding, Oral and Nasal Suctioning, oxygen therapy, Catheterisation and more.

Neurological conditions are diverse and complex, with hundreds of known conditions, including Multiple Sclerosis (MS), Huntington’s Disease, Motor Neuron Disease, Cerebral Palsy, or the result of an event such as a Stroke.

The care requirements for these conditions can be intricate and progressive, often requiring specialist services. We train staff to be able to manage both the physical and cognitive challenges that come with these conditions, allowing us to care for numerous other neurological conditions that require similar care.

People diagnosed with neurological conditions may need to make significant lifestyle changes due to the impact on their physical abilities and cognitive functions, such as perception, memory, judgement, and reasoning. This can lead to decreased independence and a need for more physical and emotional care and support daily. Many neurological are progressive in nature, and At Home Care is able to help our clients manage their deteriorating condition in collaboration with other external support services such as palliative care.
