A highly skilled and experienced team

As a specialised, high-care NDIS service provider, our entire team has extensive experience in supporting individuals with complex care needs. This includes everyone from our Support Workers and Nurses to our in-house trainers, schedulers, and leadership team. We are well-versed in taking care of the complex side of your care, so you can focus on the simple things.

Our Support Workers and Nurses receive specialised in-house training to ensure they have the skills and confidence to deliver high quality, high-care disability support. We can even provide clinical training specific to your individual care needs.

Nursing Care Plans and 24/7 clinical nursing support

When you join At Home Care, one of our Nurses gets to know you and works with you to develop a nursing care plan, which details your specific personal care needs and preferences. From here, we identify any individualised training your care team may require and deliver this training in-house.

Your care and nursing team also has access to 24/7 on-call support from our Perth-based clinical nursing team while on-shift, so you have the confidence of clinical nursing support from the comfort of your home.

At-home support for Ventilator Dependent Quadriplegics (VDQs)

At Home Care is an official provider of the Ventilator Dependent Quadriplegic Community Care Program (VDQ-CCP). Our expert team of ventilator-competent Support Workers and Nurses is led by our Head of Nursing and Care Services, Dee Donachie. Dee has over 30 years’ experience as a Spinal Nurse in acute, rehabilitation and community settings, and managed Fiona Stanley’s Ventilator Dependent Quadriplegic Program between 2006-2016.

Our Support Workers are trained to provide personal home care services for those with complex high care needs. We provide additional training to ensure exceptional quality, client-focused and specialised disability support services across Perth and WA. All Support Workers are managed by our Care Team Coordinators who have a background in care, so they fully understand and empathise with both their team and our clients. Every member of the team has access to our Clinical Nurses for additional guidance and supervision, as required.

At Home Care is experienced in transitioning clients from hospitals and residential care to the community in both local and rural settings. We also have extensive experience in transitioning and caring for quadriplegics and individuals who are ventilator-dependent.
