With At Home Care, we’re with you at every stage. Our disability and complex care services for teenagers and young adults ensure you have the help you need whenever you need it, and you can always count on us.

Our nurse-led approach provides additional support, including hospital transfer assistance, training, and funding, to help reduce the pressure on you as a parent or guardian.

First, you need to make sure we’re the right organisation for you and your child.

We understand that navigating your child’s care can be overwhelming, so we start by asking the right questions. We’ll gather information about what you need from the level of care, timeframes, documentation, and funding. These discussions will help us understand how we can best support your child and help you ensure we’re the right fit for you.

A clinical nurse will work with you through the process to clarify your goals, ensure your child’s needs are met, and work with your team. We take the time to listen to your concerns and define your child’s care goals.

This stage is all about building a strong foundation for care, training, and your team. Together, we’ll dive into your child’s medical needs, goals, and what they love to do. We’ll also talk about what success looks like for both you and your child. This might include going to university or TAFE, learning new skills, being active in decision-making, or learning how to be more independent.

With these preferences and ideas in mind, we’ll create a plan that makes you feel heard and understood.

We’ll build a team around your child’s routine and care needs. From training to recruitment, our focus is on finding compassionate individuals with the right skills and personalities, so you can feel at ease and confident in your home.

We know that this journey can be a lot to navigate, but you’re not alone! You’ll have a dedicated clinical nurse and a friendly engagement contact who will check in with you regularly. They’re always here to help and support you and your child every step of the way.

Nursing-led complex care

Every aspect of care, from delivery to training and service design, has been reviewed by our nursing team and clinical governing body.

This ensures that all aspects of care are evidence-based, medically sound, and of the highest standard.

Our in-house nursing team specialise in complex care, spinal cord injuries, and acquired brain injuries.

They must ensure their knowledge remains current through hospital-level training, renew their Australian Health Practitioner Regulation Agency (AHPRA) Registration annually, and complete 20 CPD hours each year.
