Paediatric support services for children with complex care needs

We understand it can be difficult leaving your child in the care of someone who is not you and outside your family circle, especially when your child has a disability. That’s why our paediatric in-home nursing care is flexible and tailored to your child and family’s needs, with the level of disability support decided by you.

Our family-centred approach

We’re here to work closely with you to enhance your child’s quality of life, providing high-quality care in the comfort of your home, at school, and within your community. Your family’s comfort and happiness are our top priorities.

First, you need to make sure we’re the right organisation for you and your child.

We understand that navigating your child’s care can be overwhelming, so we start by asking the right questions. We’ll gather information about what you need from the level of care, timeframes, documentation, and funding. These discussions will help us understand how we can best support your child and help you ensure we’re the right fit for you.

A Paediatric Clinical Nurse will work with you through the process to clarify your goals, ensure your child’s needs are met, and work with your team. We take the time to listen to your concerns and define your child’s care goals.

This stage is all about building a strong foundation for care, training, and your team. Together, we’ll dive into your child’s medical needs, goals, and what they love to do. We’ll also talk about what success looks like for both you and your child. With your preferences and ideas in mind, we’ll create a plan that makes you feel heard and understood.

We’ll build a team around your child’s routine and care needs. From training to recruitment, our focus is on finding compassionate individuals with the right skills and personalities, so you can feel at ease and confident in your home.

We know that this journey can be a lot to navigate, but you’re not alone! You’ll have a dedicated clinical nurse and a friendly engagement contact who will check in with you regularly. They’re always here to help and support you and your child every step of the way.

Why choose nurse-led paediatric care for children with complex disabilities

Our in-house paediatrics team specialise in caring for medically complex children. They must ensure their knowledge remains current through hospital-level training, renew their Australian Health Practitioner Regulation Agency (AHPRA) Registration yearly, and complete 20 CPD hours annually.

A nurse-led approach provides additional support from hospital transfers, training, and funding support, reducing the pressure on you as a parent or guardian.

With At Home Care, we’re with you at every stage. Our reliable paediatric support services ensure you have the help you need, whenever you need it, and you can always count on us.

Nursing-led

Every aspect of your child’s care, from delivery to training and service design, has been reviewed by our nursing team and clinical governing body. This ensures that all aspects of care are evidence-based, medically sound, and of the highest standard.

Tailored-approach

There’s no one-size-fits-all approach to our operations. We take the time to understand your child’s likes, dislikes, and requirements to ensure that you feel confident in the care they receive.

Specialised children’s services

While we may not reach everyone, we are dedicated to excelling in our specialised services. We refine our knowledge and skills to provide the best care for children with complex care needs.

Hospital-to-home collaboration

We assist children and families who might otherwise need hospitalisation to transition to living at home and engaging in the community, with support available 24/7. Our approach is open and collaborative, involving you and your child’s broader team.

Quality and safety

We aim to set a new standard for personalised community care, with quality and safety at the forefront of our services. At AHC, we are committed to strong governance rooted in six core principles: accountability, transparency, integrity, stewardship, efficiency, and leadership. Together, these values drive us to make a meaningful impact in the lives of others.
