In-Home and Complex Disability Care Specialists

At Home Care specialises in providing complex, individualised care and nursing solutions to assist people living with disabilities in building and retaining their capacity and independence.

Complex Care Specialists

At Home Care delivers personalised care and nursing solutions to assist people living with complex disabilities in building and retaining their capacity and independence.

We chose to focus on delivering reliable support services to individuals with complex physical disabilities and their loved ones. We identified a need for hospital-quality care that allows individuals to stay in their homes. Our services range from in-home nursing to dedicated support, ensuring everyone can live their fullest life, whether they need help with daily tasks or professional care for chronic conditions.

By providing around-the-clock support from our highly trained nurses and support workers, we enable individuals who would otherwise face prolonged hospitalisation to transition comfortably back into their homes and actively engage with their communities.

Quality care is crucial, which is why At Home Care’s services and training are designed, reviewed, and maintained by a Clinical Nursing team with decades of experience in complex disability care.

Our distinctive service model and governance have been recognised as a leading approach. As a result, we are proud to have been appointed to the panel for the Ventilator Dependent Quadriplegic Community Care Program (VDQ-CCP) and the Provision of Community Support for Medically Complex Children (Children and Adolescent Health Services) at Perth Children’s Hospital.

Comprehensive training for all our care team members is at the heart of what we do best. Each valued team member is trained to provide person-centred support, following an individualised care plan tailored to your specific needs.

Hospital-to-home collaboration

We assist individuals who might otherwise need hospitalisation to transition to living at home and engaging in the community, with support available 24/7. Our approach is open and collaborative, involving everyone in your broader team.

Quality and safety

We aim to set a new standard for personalised community care, with quality and safety at the forefront of our services. At AHC, we are committed to strong governance rooted in six core principles: accountability, transparency, integrity, stewardship, efficiency, and leadership.
